| dc.description.abstract | A analgesia multimodal, incluindo bloqueios interfasciais como o bloqueio do quadrado lombar (QLB), tem sido investigada na medicina veterinária como alternativa ao uso exclusivo de opióides. Na medicina humana, a associação de sulfato de magnésio aos anestésicos locais têm demonstrado prolongar a duração do bloqueio e potencializar a analgesia; entretanto, em cães, as evidências ainda são limitadas. Desta forma, o presente estudo prospectivo, randomizado, cego, teve como objetivo avaliar o efeito analgésico da bupivacaína isolada ou associada ao sulfato de magnésio (MgSO4) no bloqueio do quadrado lombar em cadelas submetidas à ovariectomia. Quarenta e cinco cadelas foram sedadas com a associação de acepromazina (0,03 mg/kg) à meperidina (3 mg/kg), por via intramuscular. A indução e manutenção anestésica foi feita com propofol (dose-efeito) pela via intravenosa. As cadelas foram distribuídas em 3 tratamentos (n=15): O grupo QLB controle (GS) recebeu NaCl 0,9% 0,5 ml/kg/ponto; O grupo QLB bupivacaína (GB) recebeu 0,5 ml/kg de solução 50:50 de Bupivacaína a 0,5% + NaCl 0,9% por ponto; já o grupo QLB bupivacaína-magnésio (GM) recebeu 0,5 ml/kg solução 50:50 de bupivacaína 0,5% + MgSO4 10% por ponto. Durante a cirurgia a suplementação analgésica foi realizada com remifentanil de acordo com os parâmetros cardiovasculares (incremento de 20% na frequência cardíaca e/ou pressão arterial, em relação à mensuração no tempo de estabilização). As variáveis mensuradas foram: frequência cardíaca e respiratória, pressão arterial, temperatura, saturação periférica de oxihemoglobina, concentração de dióxido de carbono no ar expirado, requerimento de remifentanil, requerimento de propofol, tempo até o primeiro resgate analgésico, número de resgates com morfina, número de cadelas resgatadas, avaliação da dor por meio de escala analógica visual interativa (IVAS) e escala composta reduzida de Glasgow (GCMPS-SF) e grau de sedação. As análises estatísticas foram realizadas com o software GraphPad Prism versão 6.0 e o grau de significância estabelecido para cada teste de 5% (P<0,05). Os resultados não apontaram diferença entre os grupos para os escores de dor (IVAS e GCMPS-SF) e sedação entre os grupos (P>0,05). Entretanto, para o escore GCMPS-SF quando comparado ao tempo basal (TB), o grupo GM foi o único a não apresentar diferença significativa entre os tempos avaliados após uma hora de avaliação (T1h). Em relação ao requerimento intra-operatório e pós-operatório de analgésico não houve diferença entre os grupos. Entretanto as cadelas do grupo GS (130 ± 60,8 minutos) necessitaram resgate com morfina mais rápido que o grupo GB (450 ± 75,5 minutos) (P<0,05). Com relação a frequência cardíaca, foi observada diferença significativa entre o grupo GS e o grupo GB na incisão de pele, tendo o grupo GS valor superior (P<0,05). Entretanto, a pressão arterial não acompanhou essa diferença entre os grupos. Sendo assim, a partir dos resultados encontrados, conclui-se que o tratamento com bupivacaína e bupivacaína e MgSO4 administrados no bloqueio do quadrado lombar não apresentaram resultados robustos que comprovem uma melhor analgesia durante o período intra-operatório e pós-operatório. | pt_BR |

| dc.description.abstractOther | Multimodal analgesia strategies, including interfascial plane blocks such as the quadratus lumborum block (QLB), have been increasingly investigated in veterinary medicine as alternatives to systemic opioids. In human medicine, the addition of magnesium sulfate to local anesthetics has been shown to prolong block duration and improve analgesia. However, evidence in dogs remains scarce. Thus, the present prospective, randomized, blinded study aimed to evaluate the analgesic effect of bupivacaine alone or in combination with magnesium sulfate (MgSO4) in quadratus lumborum block in bitches undergoing ovariohysterectomy. Forty-five bitches were sedated with the combination of acepromazine (0.03 mg/kg) and meperidine (3 mg/kg), intramuscularly. Anesthesia was induced and maintained with intravenous propofol (dose-effect). The bitches were distributed into 3 treatments (n=15): QLB control group (GS): 0.5 ml/kg/point of 0.9% NaCl; QLB bupivacaine group (GB): received 0.5 mL/kg of a 50:50 solution of 0.5% bupivacaine and 0.9% NaCl per point; QLB bupivacaine-magnesium group (GM): received 0.5 mL/kg of a 50:50 solution of 0.5% bupivacaine and 10% MgSO4 per point. During surgery, analgesic supplementation was performed with remifentanil according to cardiovascular parameters (20% increase in heart rate and/or blood pressure, in relation to the measurement at stabilization time). The variables measured were: heart and respiratory rate, blood pressure, temperature, peripheral oxyhemoglobin saturation, carbon dioxide concentration in expired air, remifentanil requirement, propofol requirement, time to first analgesic rescue, number of morphine rescues, number of rescued bitches, pain assessment using interactive visual analog scale (IVAS) and reduced Glasgow composite scale (GCMPS-SF) and degree of sedation. Statistical analyses were performed using GraphPad Prism software version 6.0 and the significance level established for each test was 5% (P<0.05). The results showed no difference between the groups for pain scores (IVAS and GCMPS-SF) and sedation between the groups (P>0.05). However, for the GCMPS-SF score compared with TB, the GM group was the only one that did not show a significant difference among the time points assessed after one hour of evaluation. Regarding the intraoperative and postoperative analgesic requirements, there was no difference between the groups. However, the bitches in the GS group (130 ± 60.8 minutes) required rescue with morphine faster than the GB group (450 ± 75.5 minutes) (P<0.05). Regarding heart rate, a significant difference was observed between the GS group and the GB group at the skin incision, with the GS group having a higher value during the intraoperative period (P<0.05). However, blood pressure did not follow this difference between the groups. Therefore, based on the results found, it is concluded that treatment with bupivacaine and bupivacaine and MgSO4 administered in the quadratus lumborum block did not present robust results that prove better analgesia during the intraoperative and postoperative period. | en |
